This package integrates foundational specialized training for our caregivers supporting clients with early to moderate dementia or diabetes management needs, ensuring they have the skills to provide appropriate and knowledgeable care.

TerraRosa Total Wellbeing and Family Relief™

 Our premier, all-inclusive package offering the highest level of proactive support and care management. This package ensures clients with complex needs, such as advanced dementia or those requiring intensive post-hospitalization transitional care, are matched with our most experienced W-2 caregivers who have completed advanced specialized training modules in programs like "TerraRosa Connections™" or "Post-Hospitalization Support."

Our Integrated Specialized Care Expertise

Our Specialized Care Programs offer additional expertise and are integrated into package tiers, enhancing their value.

• TerraRosa Connections™ Dementia & Alzheimer's Care: Person-centered strategies focusing on creating safe and familiar environments, promoting engagement through meaningful activities tailored to cognitive abilities, offering compassionate behavioral support and redirection techniques, and providing comprehensive education and respite for family caregivers.
• TerraRosa WellSpring™ Diabetes Support: Dedicated assistance with adherence to physician-prescribed dietary guidelines, planning and preparing diabetes-friendly meals, providing timely medication reminders (non-administration), encouraging appropriate physical activity, and recognizing critical signs for effective diabetes self-management support at home.
• TerraRosa SecureSteps™ Fall Prevention & Mobility Program: Proactive support to enhance safe mobility within the home, reduce fall risks through comprehensive home safety awareness and observational assessments, assist with prescribed exercises, and ensure proper use of assistive devices, empowering seniors to move with greater confidence.
• Post-Hospitalization & Transitional Care Support: Dedicated assistance to ensure a smooth, safe, and comfortable recovery at home following a hospital stay or rehabilitation. This includes support for adhering to discharge plans, medication reconciliation reminders, assistance with ADLs during recovery, and close monitoring for any signs of complications to help prevent readmissions.

Additional Valued Services

• Respite Care: Providing temporary, reliable, and skilled relief for family caregivers, allowing them essential time to rest, recharge, and attend to personal needs, knowing their loved one is receiving excellent, uninterrupted care.
• Overnight Care & 24-Hour Care: Ensuring continuous, around-the-clock support and safety. (Per Diem Rate: $1,100 per day).
• Client Wellness Check: Proactive, in-person visits by our Care Coordinators (Robert or Priscilla initially) for quality assurance and personalized attention outside of regular caregiver shifts. ($75 per visit).
• Personal Care Assistance: Dignified and respectful support with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, incontinence care, and mobility assistance.
• Companionship & Social Engagement: Providing meaningful human interaction, engaging conversation, participation in hobbies and interests, accompanying on social outings, and offering vital emotional support to combat loneliness and promote mental well-being.
